Here we answer frequently asked questions from our patients.

What are the approximate costs of orthodontic treatment and when should I make an appointment for my child?

That depends what has to be corrected, what kind of braces or retainers are to be used, whether a public health insurance plan covers the costs… it is impossible to give a general answer to this question. The total costs can be anywhere between zero and several thousand euros.

The ideal time for the beginning of orthodontic treatment should be chosen such that as good a result as possible can be achieved in the shortest possible time. This depends on what has to be corrected, and whether waiting longer could mean negative consequences. The only way to assess the right time is to have an orthodontist examine the dental situation.

Does malocclusion always have to be treated with braces?

No, for many malocclusions there are removable alternatives like Invisalign or similar treatments. However, keep in mind that even these removable retainers and brackets should be worn for at least 22 hours a day, although you can "be free when it really matters".

Are malocclusions hereditary?
Crooked teeth are caused by what are called “polygenetic factors”, meaning that in addition to hereditary factors, which can be quite dominant, habits like lip biting, thumb sucking, and incorrect swallowing patterns with the tongue, to name just a few, can play a major role in the emergence of malformations. Tongue function and swallowing play a very important role. Did you know that the average person swallows 1500 to 2000 times a day?
Is there a way to take precautions so that children will not need treatment?
See the orthodontist early to recognize any need for speech therapy or habits that need to be addressed (because of negative effects on tooth or jaw positioning).
Do malocclusions really come from sucking?
Yes, unfortunately, when children continue sucking on fingers, thumbs, or pacifiers after their third birthday, it usually results in permanent deformation of the jaw and shifted teeth. If a child can break the habit before his or her third birthday, growth of the jaw and teeth usually returns to normal. By the way, it is easier to break a pacifier habit than thumb sucking, since the pacifier can be exchanged for something else like a comfort blanket. Motivation and rewards are usually the most successful approaches.
How does optimum dental hygiene work with fixed braces?

Teeth with fixed braces are more difficult to keep clean. To remove all impurities, the brush has to be applied at a slanted angle around the brackets. We also recommend interdental brushes to our patients so that they can reach between the brackets. What are the most modern/latest treatment methods?

The most modern treatment methods in orthodontics make use of what are called CAD/CAM, and thanks to digitally supported treatment planning, they often present significant advantages in terms of precision and treatment length. Examples of this include suresmile treatment and Invisalign therapy. What is more, such methods allow the braces or brackets to be practically or even completely invisible. In addition to these techniques, today it is possible to use mini-screws anchored in the jaw to achieve tooth repositioning that was never possible before, or at least not without headgear.

How early should malocclusions be treated?
That depends on the malocclusion: For severe malocclusions and jaw misalignments, and for those which can have detrimental effects on jaw growth, treatment can be beneficial at the age of three or four. Otherwise it is usually sufficient to see the orthodontist at the age of around nine to determine whether treatment is necessary, and if so, when it should begin.
How do you deal with children who are afraid of the dentist/orthodontist?
In such cases the best approach is to spend the first appointment just meeting and talking with the children. In many cases no comprehensive examination is performed until a later appointment, as the purpose is to win the child’s trust.
Up to what age should/must a patient wear braces or retainers?
Malocclusions can be corrected at any age. If seniors want to display a perfect smile at their afternoon socials, they can still have their teeth straightened at 90.
Which is better: fixed braces or removable retainers?
There is no definite answer to this. The braces or retainer that allows the flexibility a patient needs and fits in with his or her everyday routine is the one to choose.
Do braces increase the risk of tooth decay?

Yes, every kind of braces or retainer presents an additional risk of cavities. This why it is so important to keep orthodontic treatment as short as possible.

Is treatment with fixed braces painful?

Not continuously. At the beginning teeth may be sensitive when you bite down, and toothaches can occur, but usually any pain disappears after a week or so. Later, some pressure may be felt when wires are changed, but this pain is generally much less than at the beginning of treatment. The amount of pain also depends on the kind of braces.

Is it always necessary to remove teeth to make room for fixed braces?
At ADENTICS only around 5% of our patients have to have teeth removed to make room for braces. There are often other ways to make sufficient space. A Hollywood smile needs as many teeth as possible.
Do the white marks from fixed braces stay there for a lifetime?
The whitish marks are not a direct consequence of braces, but come from an early form of tooth decay that occurs during treatment due to deposits remaining on the teeth for too long. To make these traces less noticeable, discoloured enamel can be infiltrated with a special plastic (caries infiltration with ICON). Extremely shallow lesions can often be polished off. Bleaching can also help make the white marks less visible.
How can I find a reliable orthodontist I can trust? Do I have to visit my local orthodontist, or can I choose any one I like?

In Germany, you have the free choice of physicians. Nevertheless, keep in mind that since orthodontic treatment entails regular appointments, it is advisable to find a practice nearby. At ADENTICS, we even have patients fly in for appointments.
